Patient Responsibilities

As a patient, you agree to:

• Provide accurate and complete health information
• Attend scheduled appointments or provide advance notice of cancellations
• Follow prescribed treatment plans and medication instructions
• Pay for services rendered according to your insurance plan or fee schedule
• Treat our staff and other patients with respect and courtesy

Appointments and Cancellations

We request at least 24 hours' notice for appointment cancellations. Repeated missed appointments or late cancellations may result in a fee or limitation of services.

Billing and Payment
